What does it mean when people speak of "regenerative medicine" and what implications might be present for patients undergoing regenerative medicine treatments?

Regenerative medicine is a new field of medicine that combines the basic science experience from geneticists, cell biologists and immunologists with the clinical skills and understanding from orthopedics, physiatry, plastic surgery and even internal medicine so that the patient may maximize their bodies potential to optimize local and system wide tissue injury and degeneration. This new field of medicine is trying to more fully understand how our body naturally heals from injury so that we can apply the best and most favorable aspects of healing to areas in our body that need it, without having to undergo surgery.

Over 20+ years of recent basic science and some, more recent, prospective human trials has shown the potentially significantly positive effects when patients are provided regenerative medicine treatments. In all cases, the potential benefits to the patient are maximized when the patient is overall optimized for the treatment (eating well, hydrating well, sleeping well, supplementing well and minimizing certain types of medications) and the patient is appropriately indicated for the treatment (the doctor has correctly identified the patient would likely benefit from this specific treatment).

Minimally-invasive, Non-surgical Procedures

The most common forms of regenerative medicine treatments can be divided into 2 groups – local versus systemic. In the local group of treatments, there is a wide variety of options. Platelet rich plasma injections, amniotic fluid injections and stem cell injections provide the most robust local regenerative medicine results currently available. Platelet rich plasma injections provide a strong signal to the surrounding cells to initiate a healing response. PRP is analogous to our city’s first responders, they stop the bleeding and recruit the specialists to heal the injuries.

Amniotic fluid injections are analogous to PRP injections but are considered more robust given the higher concentrations of proteins in the amniotic fluid that are particularly helpful in cell turnover and healing. Amniotic fluid injections also contain an important joint lubricant called hyaluronic acid. Stem cells are those nascent cells that are awaiting stimulation to migrate to a site and transform into local reparative tissue. Stem cells have the greatest potential to heal local trauma and disease. These cells are found in the highest numbers in our fat but also are found in good concentration around our pelvis bone.

Mesenchymal stem cell injections, derived from either our bone marrow or fat stores, provides the most robust, non-surgical method to heal injuries and relieve arthritic conditions. These stem cell injections can be applied to all the same target sites as PRP and amniotic fluid. Because the injections contain living pluripotential cells (cells that can transform to adapt to the needs of its local environment), the potential for healing, even incomplete structural defects, is very promising. Impressive results in alleviating pain, restoring local healthy tissue and suppressing inflammation have occurred in joints, tendons and ligaments with prior trauma, surgery or degenerative arthritis conditions.
